Note:  Project MALE's Program Year is typically from October - May.  

Registration Materials:

​The Student Registration Form is to be completed by the parent/guardian before any child is considered to be registered for Project MALE.  
​

Form 1:  Student Registration Form

This form is completed by the parent or guardian and is used to enroll the student into the Program.  This form is also used to authorize pick up of the student by designated individuals.

Form 2:  Photo and Video Release Form

This form is completed by the parent or guardian and authorizes Project MALE to publish the photographs and videos taken of the student for use in the Project MALE printed publications, website, and training.

Form 3:  Student Code of Conduct

This form outlines the five standards of conduct offered as guides to the development of Classroom Expectations for students enrolled in the program.  The parent or guardian also lists any allergies of the student that Project MALE should be aware of.

Form 4:  Field Trip Permission Form
This form is completed by the parent or guardian and confirms permission for the child to participate in Project MALE field trips.  Transportation will be provided.  By completing and signing this form, the parent or guardian agrees to release and hold harmless Project MALE and its representatives, employees, and agents from any and all claims, costs, suits, etc.
